If you are eligible to adjust your status to permanent resident, please use the Permanent Residency Application Packet. You will need to file the following applications:
I-485
I-130
I-765 (to obtain a work permit; optional)
G-325A
I-864 (if needed)
I-693
The I-693 must be filled out by a civil surgeon authorized by the Bureau of Citizenship and Immigration Services. To find an authorized civil surgeon in your area to to www.uscis.gov/graphics/exec/cs/index.asp

The I-765 may be filed online. You may do so by going to
