We are now accepting Summer Financial Aid Applications. Please see below for a link to the form.

Important note about form submissions:

You may send your forms to the financial aid office of the LDSBC in one of the following ways:

  • Deliver in person - 2nd floor of the LDSBC

  • DO NOT submit a verification form unless requested.
  • Please review the "To Do List" in your Student Center to determine if this is required.
  • Not all students are required to submit this form to the financial aid office.

2013-14 Forms (Fall 2013 - Summer 2014)

Information Sheet 2013-14

  • In addition to the 2013-14 FAFSA, this form is required by the financial aid office.
  • Complete this form after you have completed the 2013-14 FAFSA and have been admitted to the college.
  • Submit this form to the financial aid office.

Helpful Hints to Complete the 2013-14 FAFSA

  • This is a brand-new form intended to provide assistance to students filling out the 2013-14 FAFSA.

Additional Stafford Request Form 13-14

  • This form is required if you are requesting a loan in addition to the loan you accepted online, or of you are requesting a loan you previously turned down.
  • There is no need to submit this form unless you have already been offered financial aid in your Student Center (MyBC online).

Parent PLUS Loan Application 13-14

  • This application is for parents of dependent students.
  • Please visit studentloans.gov and the Student Loans section of our web site for more information. The student must also submit the FAFSA. 

General Forms 2013-14

General Appeals Form 13-14

  • If you have been disqualified from receiving financial aid due to not meeting the Satisfactory Academic Progress requirements, you may appeal the disqualification if you have unusual circumstances beyond your control.
  • Submit this form along with all of the requested documentation on the form.

Authorization Form 2013-14

  • This form is used to authorize or deny payment of non-institutional charges with your financial aid (such as health plan charges, late fees, library fines, etc.).
