2014-15 School Year Forms are Now Available.

Important note about form submissions:

You may send your forms to the financial aid office of the LDSBC in one of the following ways:

  • Deliver in person - 2nd floor of the LDSBC

2014-15 Verification Forms

Please double-check your "To Do" List in your Student Center to ensure you are completing the correct form. Please call our office if you have any questions.

  • DO NOT submit a verification form unless requested.
  • Not all students are required to submit this form to the financial aid office.
  • Please review the "To Do List" In your Student Center to determine if this is required.

2014-15 Forms (Fall 2014 - summer 2015)

Information Sheet 2014-15 (PDF)

  • Complete this form after you have completed the 2014-15 FAFSA and have been admitted to the college.
  • Submit this form to the financial aid office.

Helpful Hints to Complete the 2014-15 FAFSA

  • This form intended to provide assistance to students filling out the 2014-15 FAFSA.

Additional Stafford Request form 14-15

  • This form is required if you are requesting a loan in addition to the loan you accepted online, or of you are requesting a loan you previously turned down.
  • There is no need to submit this form unless you have already been offered financial aid in your Student Center (MyBC online).

Parent PLUS Loan Application 14-15

  • This application is for parents of dependent students.
  • Please visit studentloans.gov and the Student Loans section of our web site for more information. The student must also submit the FAFSA.

Request for Special Consideration 14-15

This form can be used if your (or your parent's) current financial situation is not accurately reflected by your 2013 tax information. 

General Financial Aid Forms

Appeal of Satisfactory Academic Progress (SAP)

  • If you have been disqualified from receiving financial aid due to not meeting the Satisfactory Academic Progress requirements, you may appeal the disqualification if you have unusual circumstances beyond your control.
  • Submit this form along with all of the requested documentation on the form.

Financial Aid Academic Plan for SAP Appeal

This form is only required for one type of SAP appeal. Refer to the other SAP form for details.
